How
it works.
••••••••••••••••••••••••••••••••••••••••••••••••
1. Injury occurs while at work.
Employer sends employee to WorkCares at Coordinated
Health
••••••••••••••••••••••••••••••••••••••••••••••••
2.
Employee Comes In To Nearest Facility For Care.
Medical Assistants triage and prepare
the patient for immediate attention.
••••••••••••••••••••••••••••••••••••••••••••••••
3.
While patient is seen by a Physicians Assistant, the
Case Manager speaks with the patient, and speaks with
medical staff about the situation.
Also, the employer and the insurance carriers are contacted.
••••••••••••••••••••••••••••••••••••••••••••••••
4.
A Physician, Physician’s Assistant, or Nurse Practitioner
tends to the patient, evaluates the injury, and recommends
a proper treatment plan.
The case manager documents the information
and consults the physician about the patient’s
activity status and ability to return to work.
••••••••••••••••••••••••••••••••••••••••••••••••
5.
Patient is discharged with comprehensive treatment plan.
Case manager contacts employer to inform them of the
situation and explain the next steps that need to be
taken for the individual. If the individual requires
a physician, an appointment is available the same day.
