The Highland Avenue office
opened its doors for the first time in April of 2005.
It currently houses The Imaging Center, The Hand and
Wrist Center, The Plastic Surgery Center, and one location
of The Back and Neck Center. Also located at the 2300
Highland office on the second floor is Coordinated Health’s
Medical Careers Institute and the offices of Lehigh
Valley Retinovitrious Associates. The lower level is
the home of Fadici’s, Café 2300.

From Easton and Phillipsburg - Take
Route 22 West to Route 512 North. Turn right onto Route
512 and follow 1/4 mile to Highland Ave. Follow straight
to CH Surgical Specialty Center and Medical Office Building.
From
Allentown and Bethlehem – Take Route
22 East to Route 512 North. Turn right onto Route 512
and follow 1/4 mile to Highland Ave. Follow straight
to CH Surgical Specialty Center and Medical Office Building.
From the Poconos– Take Route
33 South to Route 22 West. Follow to Route 512 North.
Turn right onto 512 and follow 1/4 mile to Highland
Ave. Follow straight to CH Surgical Specialty Center

Fadici’s Café 2300
“Food you can feel good about”
Located in the lower level of the 2300 Highland Avenue
building is Fadici’s, Café 2300. Fadici’s
Gourmet specializes in a Mediterranean menu, touted as
a healthy way to cut fat and eat better. Their menu focuses
on Greek, Italian and Spanish foods along with American
favorites.
Fadici’s is not owned or affiliated in any way
with Coordinated Health except as an independent contractor
that provides food services to some Coordinated Health
entities.

Jonathan Swart, MS, ATC
 Jonathan
Swart, MS, ATC is
the manager of Coordinated Health’s Highland Avenue
Facility. He oversees the operations for
the 47,000 square foot facility and is a practice manager
for The Back and Neck Center, The Imaging Center, The
Hand and Wrist Center, and The Plastic Surgery Center.
Jon is a 1992 graduate of East Stroudsburg University
where he was a Cum Laude graduate receiving his Bachelor
of Science in Athletic Training. He then went on to get
his Master of Science from West Virginia University. While
at WVU he was a graduate assistant athletic trainer with
the Mountaineer football team who was a participant in
the 1994 USF&G Sugar Bowl. Jon practiced for ten years
as a Certified Athletic Trainer in the Collegiate setting.
Institutions included were Seton Hall University and most
recently his Alma Mater, East Stroudsburg where he was
the Associate Athletic Trainer and a clinical instructor
in ESU’s accredited athletic training program. In
January of 2003, Jon joined the Coordinated Health team
where he continues to grow with the company. Personally,
Jon is a resident of Hanover Township where he lives with
his wife Carolyn and his two daughters, Julia and Mara.
