What to Expect Before Therapy

Be sure to bring the therapy prescription given to you by your physician. This prescription tells us your diagnosis and what your doctor wants done in therapy. Patients referred by Coordinated Health a physician do not need to worry about this part - your prescription is available via your electronic health record.  Please also bring a list of your current medications

Before you arrive for your initial evaluation, our staff takes care of verifying your insurance, co-pays, co-insurance, deductible and number of visits your insurance allows. If necessary, we obtain referrals from your primary care physicians. If we encounter any problems with referrals or your insurance verification, we will contact you prior to your initial evaluation.
